The short answer

A false negative is a real failure that an evaluator accepts or fails to flag. Inspect accepted outputs as well as alerts, use independently labeled challenge cases, and distinguish detection recall from real-world failure frequency. A detector that looks quiet may simply be missing the cases that matter.

What to take away

  • Audit accepted outputs as well as alerts.
  • Separate challenge-set recall from prevalence.
  • Pair material mutations with acceptable controls.

Define a missed failure in observable terms

Scikit-learn’s metrics documentation distinguishes recall from precision. For an evaluator whose positive label means failure, recall asks how many reference failures it detects. That quantity differs from how often the deployed product fails and from how many alerts are worth reviewing.

Write a failure taxonomy with concrete evidence requirements. A citation mismatch, an omitted exception, and an unauthorized state change need different observations. Do not use one vague unacceptable label when the reviewer must inspect a source passage for one case and a tool’s final state for another.

Sample accepted outputs deliberately

Review a representative sample of outputs the evaluator accepted, and add targeted slices where misses would be consequential. Keep the representative audit and the enriched challenge set separate. An intentionally difficult set can test detection capability without estimating the frequency of failure in normal traffic.

Record why each accepted output was selected. If a user complaint triggered the review, preserve that pathway. Combining complaint-driven failures with random accepted outputs and reporting one prevalence number would misrepresent the sampling process, even if every individual failure label is correct.

Construct minimal missed-failure controls

Start from a correct answer and introduce one material change: reverse a relationship, remove an essential qualification, or attach a citation that does not support the claim. Keep other properties stable so the expected failure is unambiguous. These are authored controls; they are not observed model errors.

Pair each mutated output with its original. The evaluator should distinguish the material defect while accepting the correct control. If both receive the same acceptance decision, inspect what evidence the evaluator was actually given and whether its rubric includes the relevant requirement before increasing model complexity.

Report recall and investigate the blind spot

Calculate detected reference failures divided by all labeled reference failures, with counts by failure type. When no reference failures exist in a slice, recall is undefined; report the absence of evidence instead of perfect detection. Preserve indeterminate judgments and missing evaluator outputs separately.

For each confirmed miss, determine whether the cause is missing context, an incomplete rubric, a weak threshold, or an evaluator reasoning error. Retest an amended evaluator on held-out controls and ordinary acceptable answers. A rule that catches the new failure by rejecting every answer creates a different operational problem rather than a useful detector.

Limits of the evidence

The mutation cases and audit procedure are proposed methods. No evaluator was measured here. Label quality, sample selection, and access to tool or source evidence limit any detection estimate; a challenge suite cannot establish production failure prevalence on its own.

Common questions

Can I estimate production misses from a challenge set?

Not without a sampling model that connects it to traffic. Enriched cases reveal detector capability, while representative sampling supports prevalence estimates.

What if no failures appear in the audit?

Report the sample and uncertainty. Zero observed misses in a bounded review does not prove that the detector catches every failure.
