The short answer

Design an agent audit trail around the question of why a consequential operation occurred and what it changed. Link task intent, authenticated identity, proposal, approval where required, tool execution, and authoritative receipt. Model text can explain context, but it should not be the only evidence that an action was permitted or completed.

What to take away

  • Use stable identifiers across proposal and execution.
  • Retain authoritative decisions and receipts.
  • Exclude unnecessary sensitive prompt content.

Choose the investigation the record must answer

LangSmith documents production tracing and review workflows. Rubrex recommends defining an action-focused evidence contract before choosing which events to collect. A reviewer should be able to reconstruct the requested operation, who authorized it, which policy applied, and the actual resulting state.

Use a synthetic account plan change. The record needs task and operation identifiers, caller identity reference, old and new plan, approval reference if required, adapter version, and service receipt. The complete conversation may be unnecessary when these fields already explain the consequential transition.

Evidence: LangSmith Observability [1]

Connect records across asynchronous boundaries

Send the action through a mock queue, retry one delivery, and delay the service acknowledgment. Inspect whether the same business operation remains linked to all attempts. Separate operation identifiers from trace identifiers: a trace can end before a queued write completes, and a retry can start a new trace.

Add a denial case and a canceled task. The record should distinguish a proposed action, an attempted denied action, an admitted operation, and a confirmed commit. Logging only successful responses hides exactly the boundary failures an investigation often needs to understand.

Test completeness and sensitive-data handling

Give a reviewer the exported record without access to hidden model reasoning. Ask them to identify the approved target, actual arguments, final state, and unresolved work. Missing relationships should be recorded as evidence gaps rather than filled from the assistant’s confidence.

Insert unique synthetic markers into credentials, user text, and a tool error. Verify that restricted values are absent from the intended audit destination while stable identifiers remain usable. Include a legitimate control with enough safe context to explain the action; indiscriminately removing every field can make the record privacy-conscious but operationally useless.

Retain a usable record with an accountable owner

Accept the plan-change fixture when a reviewer can connect one authorized intention to its actual effect and distinguish denied or uncertain attempts. Assign ownership for record access, retention, export, and correction according to the organization’s requirements. This article does not prescribe a legal retention period.

Re-run the export review when queues, adapters, identity propagation, or redaction change. Check that timestamps and links survive migration to the retained format. An audit trail is useful when it supports a concrete investigation and decision; a large collection of prompt logs is not automatically an adequate action record.

Limits of the evidence

The account change and evidence contract are illustrative recommendations. No audit system or compliance control was certified. Retention, access, and legal requirements depend on the organization; documentation-based tracing features are not proof of record completeness.

Common questions

Must hidden reasoning be logged?

No. User intent, policy decisions, tool requests, receipts, and final state can provide inspectable evidence.

Is every trace an audit record?

A trace may lack identity, approval, or commit evidence needed to explain a consequential action.
